Our practice participates in formal quality measurement through Innovation Care Partners. Quality measures include Blood Pressure control, Diabetes care, Cancer screening (Breast, Colorectal), Cardiovascular Disease treatment, Kidney Health, Osteoporosis and other preventive care and coordination of care.

How we measure quality

Your care is measured through Innovation Care Partners, a Medicare-approved ACO.
This means your doctor’s care is tracked using the same standards used nationwide.

These measures look at:

  • Control of chronic conditions like diabetes and blood pressure
  • Cancer screening rates
  • Preventive care
  • Follow-up and coordination of care

  • Based on real patient outcomes
  • Compared across many practices in the ICP network

       This score was higher than large hospital-owned primary care groups in the same network.
